1. Active Business License: (varies with state) but we require a copy of your business license to review before we can fulfill your order. Once license is approved we forward a current line sheet for selections.

2.Billing & Shipping Information: billing & shipping info, proof of shipping address, copy of your business EIN along with a copy of your ID

3. ALL Items Sold By ASSORTED Dozen: You will receive 12 items per unit, minimum purchase $150

4. Invoices must be paid in full by 2pm the same day your order is placed.
